It involves a dentition of 12 teeth / crowns placed on 6 implants in one toothless jaw. All on 6 is a much more stable and long-term solution compared to All on 4. A larger number of implants support the dental bridge on its end parts (there are no “hanging” members), which contributes to a good static and a dynamic of the bridge.
This solid and resistant construction can accept stronger chewing forces and higher weight of the bridge. That is why All on 6 is mostly made of ceramic materials that are heavier and more sensitive, but have one wonderful and relieving feature – they do not change color in the mouth over time.
All on 4 and All on 6 are concepts that have emerged in the market due to different patient demand and the supply of the implant industry.
The fact is that the All on 6 concept requires more bone in the back of the jaw. In order to provide patients, who do not have enough bones, with some kind of conditionally fixed work, All on 4 was subsequently developed. The concept works, but with certain limitations in relation to All on 6.
It is important to know that even if you do not have enough bone for All on 6, there are bone upgrade procedures that will allow you this type of therapy. If you have enough bone, it does not need to be upgraded and implantation can begin immediately.
How many implants do I need?
To understand how many implants you need for one jaw, it is necessary to understand the types and characteristics of prosthetic works that are part of All on 4 and All on 6. Bridge or dentures, acrylic or ceramic teeth – this is something you will have in your mouth for a long time. Every prosthetic work has its advantages, disadvantages and limitations, and it is up to you to decide which one you want. Only after that, we make a choice of the number of necessary implants required for the desired prosthetic work.
The ceramic bridge within All on 6 is mostly done up to the position of the sixes (12 crowns). There are deviations from this rule. If you have a full dentition in the opposite jaw, if there are molars and wisdom teeth, it is logical that you will have a longer bridge on the implants in order to have the maximum chewing function. Then mostly 8 implants are needed.
In the upper jaw, we have a thinner spongy bone and mostly the work is completed with 6 to 8 implants. The bone in the lower jaw is more compact and 6 implants are enough.
Regardless of the All on 6 or All on 4 concepts, the number of implants depends on the individual situation in the mouth.
How is All on 6 fixed?
It can be fixed with screws or cementing.
All on 6 fixed with a screw is a more elegant and a better variant. It can only be removed by a dentist and can be repaired to some extent. The good thing about All on 6 on the screw is that there is no fear that the cement will get under the gingiva (it is known that cement that goes unnoticed can cause serious complications).
All on 6 can be cemented but only in situations where the structure does not go under the gingiva and the cement can be easily removed. The cemented bridge cannot be removed unless it is destroyed, and then a new one needs to be built.

How is All on 6 maintained?
Good and regular oral hygiene is necessary for all work on implants. The toothbrush alone is not enough, special brushes and dental floss for implants must be included in the cleaning. Interdental brushes are always as good as oral irrigators.
We remove the screw works in the office once or twice a year in order to clean the part of the bridge that lies on the gums. Cemented bridges cannot be temporarily removed. They are designed to allow you to clean better every day at home.
All on 6 – advantages and disadvantages
The biggest advantage of All on 6 is that it is a stable and long-lasting construction. It is made of ceramic materials that do not change color. Once determined, a certain tooth color accompanies you for many years. There is no accumulation of pigmentation or yellow discoloration.
Maintenance is much easier compared to All on 4 because the ceramic is smooth and glazed.
Not to be confused – All on 4 can also be made of ceramics, but for that certain conditions must be made, which is often not possible with this construction.
The ceramic bridge in All on 6 is gracefully minimally thicker than natural teeth and there are no speech problems or tongue discomfort.
You can get a smile on the same day (immediately after the implant placement) on the All on 6 work. It is a temporary prosthesis which, until the fusion of the implant with the bone is completed, will serve only for aesthetics and not for chewing.
The only “disadvantage” of All on 6 is that it requires a larger number of implants, more bones and a higher price compared to All on 4.
